version_14 branch

I've just committed my changes to the version_14 branch (I know we're
still calling it version 13).  A couple of notes:

The test suite takes almost twice as long to run now :(

t/multiplexer.t and t/nofork-mux.t now fail.  I think it's because
peek() exhausts the filehandle and IO::Select now thinks we can't get
any more data for the parsers.  I haven't looked into this carefully
enough to fix the bug.
